• Contacting Customer Service – please e-mail all questions to customerservice@surscan.com.

• Prescriptions – ONLY the (MRO) Medical Review Officer will contact you, if prescription information is needed. This will take place within a few days AFTER you have completed giving your specimen.

• Rescheduling – because of the number of students to be tested, please make sure your test when scheduled. If you have a conflict with the assigned date you receive, contact Customer Service as soon as possible customerservice@surscan.com.

• Directions to assigned testing site - will be e-mailed to you after you complete your drug test registration. Be sure to continue to check your e-mail for these instructions.

On the next page You must ONLY INPUT THE STUDENT’S INFORMATION in the BILLING INFORMATION. Once again, it must be the students NAME, CURRENT ADDRESS, PHONE # and E-MAIL ADDRESS even if it is SOMEONE ELSES CREDIT CARD. This is how the student’s registration for drug testing will be tracked.

School Selection: You MUST select, at this time, ALL the nursing programs you will be applying to. Only those schools, that you selected, will have access to your test results.
